If you would like to opt out of your data being used in this way, you can do so in one of two ways:

  1. By logging your preference at the National Data Opt-Out site: https://digital.nhs.uk/data-and-information/data-collections-and-data-sets/data-collections/general-practice-data-for-planning-and-research
  2. By completing the Type 1 Opt-out form at the link below and returning it to the surgery
