Patients’ Rights and Responsibilities

Patient Confidentiality

We respect your right to privacy and keep all your health information confidential and secure. We record information about you so that you can receive the best possible care and treatment. We keep this information, together with details of your care, to ensure that your doctor or nurse has accurate and up-to-date information. We sometimes need to pass on information about you to other people, such as hospitals, Herefordshire & Worcestershire Clinical Commissioning Group (CCG), or the Dept. of Work & Pensions. This is always done confidentially or by removing identifying details when they are not essential. All NHS staff have a legal duty to maintain the highest level of confidentiality about patient information.

Your personal information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care. You have a right to know what information we hold about you. If you would like to see your records, please contact the Practice Manager. Patients With Disabilities

We have suitable facilities to accommodate patients and visitors who use a wheelchair. There is ample parking and facilities are easily accessible on the ground floor. Should you require help, any member of staff will be pleased to assist you.

Making a Complaint or Leaving a Review on NHS Choices

If you wish to comment on the service provision or make a complaint, please telephone or write to the Practice Manager – Su Suehr. You can also leave a review on NHS Choices, although specific issues are more effectively dealt with through our complaints process where we can address problems directly. Zero Tolerance

This Practice supports the Government’s Zero Tolerance Zone Campaign. We ask that you treat our GP’s and practice staff properly – without the use of violence or abuse. If a patient is violent or abusive they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.
