A Message From the GP Partners About Care Navigation

We have asked the Receptionists to ask you for a little information about why you’d like a GP appointment. This is simply so they can direct you to the person most suitable to help you.

Of course you don’t have to do this, but it would help us to help you and other patients.

When you ring or ask for a GP appointment our Receptionists may ask you for some brief information about why you need the appointment. This is so that we can direct you to the most appropriate help. 

Sometimes that may be a different member of the team at Fownhope Medical Centre – occasionally it may be to someone outside the surgery e.g a Pharmacist or Optician. Be assured that we will not prevent you from booking an appointment with a GP, but seeing another person who is better suited might save you 2 trips, or could mean you get help sooner. 

This is part of a National initiative called Care Navigation which aims to make the best use of the NHS for the benefit of everyone. All surgeries in Herefordshire are taking part.
