Our Social Prescribing Service

We have a Social Prescriber available at Fownhope Medical Centre every other week. The Social Prescriber can support people to access non-medical services, information and advice that will help them take greater control of their health and wellbeing. 

The Social Prescriber will work with patients to look at what they are doing, can do or would like to do and provide ideas and support to make informed choices about their life. Together our Social Prescriber and the patient will identify any barriers and difficulties which are faced and look at overcoming these.
